IHH Healthcare Sets Sights on India's Top Three with Fortis Expansion

Malaysian hospital giant IHH Healthcare has named India a high-priority market and plans to raise its Fortis Healthcare stake to 50 percent over the medium term. Group CEO Prem Kumar Nair emphasized quality over sheer scale, targeting 10,000 beds by 2030 without compromising clinical standards. This push comes amid resolved regulatory hurdles, positioning IHH to challenge India's private healthcare leaders.

Quality-Driven Growth Strategy

IHH prioritizes high-quality beds over volume, with Nair warning against chasing numbers at the expense of care. Fortis commits ₹2,500 crore in capital expenditure over three years for brownfield expansions in Punjab, Jalandhar, Bengaluru's People Tree, and Manesar in the National Capital Region. Full integration of Gleneagles Global Hospitals brings Fortis to 36 facilities, backed by an IHH board visit to inspect operations this week.

Financial Turnaround Fuels Ambition

IHH entered Fortis at ₹170 per share in FY19; the stock now trades near ₹800, up from peaks of ₹1,000. From a loss-making entity, Fortis now posts 23-24 percent EBITDA margins, with revenue growing in the mid-teens and EBITDA expanding at mid-20s percent annually over five to six years. Beds and hospitals have increased from 27 to 36, and executives expect further stake hikes to make earnings per share accretive despite the current 31 percent holding.

Innovations in Diagnostics and Care Delivery

IHH repositions its diagnostics arm, formerly SRL and briefly Agilus, as a high-end genomics lab integrated with Fortis hospitals for precision medicine. After spending ₹50 crore on rebranding before reacquiring SRL, the unit now runs steady margins comparable to Fortis. The group advances ambulatory care for knee replacements, spinal procedures, cardiac interventions, and chemotherapy to cut costs, alongside a "true cost project" to trim hospital billing inefficiencies and investments in oncology tools like Fortis's MR-Linac.
